The Mini‑Grant Program
The Adirondack Health Foundation’s Mini-Grants Program provides small, high-impact funding awards to Adirondack Health departments, staff-led projects, and community partners who are working to improve health, wellness, and patient experience across our region.
These grants support innovative ideas that make a measurable difference — from new equipment and programming to pilot projects that enhance care delivery and community health.
Our goal is simple: empower great ideas that elevate patient care and advance our mission.

2026 Awardees
2026 Mini-Grant Awardees
The 2026 Mini-Grant recipients represent a diverse group of departments and initiatives across the health system. Funded projects include:
Patient Care
- Emergency Department – ED Fluid/Blanket Warmer Combo to enhance patient comfort and care delivery
- Pharmacy & Cardiology / Health Centers – Self-Measured Blood Pressure Program to expand patient access and improve chronic disease management
- Tupper Lake Health Center – “Reach Out and Read” program to support early literacy and community health
- Endoscopy Department – Patient Belongings Cart to improve patient experience and operational efficiency
- Endoscopy Department – Clean vs. Contaminated Storage Unit to strengthen infection control and workflow
- Nutrition & Rehabilitation Services – Adaptive Feeding Equipment to support continued nutritional health and patient independence post-discharge
- Palliative Care Program – CAPC Membership to enhance training and expand access to high-quality palliative care services
- Operating Room – Wilson Frame equipment to support surgical care and improve patient outcomes
- Outpatient Rehabilitation – High-low treatment table to enhance patient safety and therapy effectiveness
Workforce
- Education Department – Pathway Employee Appreciation Cart to support staff engagement and workforce well-being
- Information Technology – IT Training initiative to strengthen staff knowledge and system utilization
Innovation
- OB Department– Ultrasound “Butterfly” device to expand bedside imaging capabilities
- Rheumatology – Smart-G-Scote USB capillaroscopy to enhance diagnostic capabilities
